If you are new to this site, consider using the contact option on our home page to be introduced to a member of our welcome committee. Once you are welcomed, we encourage you to join. Joining the AIS-DSD Support Group can be completed in two easy steps. First, complete the membership application below. After you submit, finish by completing payment for annual dues and membership tier selection with the secure online options we have included or through regular mail. Voting membership will be granted to the following:

Family Member (One membership per family)
